  • Phoenix Health Fund membership details and bank account.

Claim via the Phoenix Health Fund member portal

  1. Log in to the Phoenix Health Fund member portal.

  2. Select Submit a claim (or the equivalent option).

  3. Upload your HaltH invoice.

  4. Click Submit.
