Your HaltH invoice (also sent via SMS and email after payment).
Phoenix Health Fund membership details and bank account.
Claim via the Phoenix Health Fund member portal
Log in to the Phoenix Health Fund member portal.
Select Submit a claim (or the equivalent option).
Upload your HaltH invoice.
Click Submit.
