Uploading Your Provider Letter

The documents required, based on your service type, to onboard with HaltH for health fund claiming.

To look up claims and process gap payments with HaltH, we require your provider letter to verify your registration details, including your full name, the effective date of the letter, and the registered practice location.

Please see below based on your service type what documents are required:

  • Exercise Physiologist

  • Physiotherapist

  • Podiatrist

  • Psychologist

Your Medicare Australia Provider Letter for the registered practice location

OR

A download of the Health Professional Online Services (HPOS) Medicare Registration Status for your registered practice location with the current date showing on the document. Please see here on how to get this document.

  • Remedial Massage Therapist

Your Medibank Private Provider Letter for the registered practice location

AND

Your current Certificate of Registration from the Association of Massage Therapists.
